INTRODUCTION  Multnomah County Dental program serves over 25,000 unique patients every year, who have about 80,000 dental visits.  Over 90% of our patients are designated as falling below the poverty level.  Approximately 35% of our dental patients are children.  We are the largest social safety net dental program in the state of Oregon.  We provide dental services including routine care, cleanings, extractions, pain relief, dentures, root canals (endodontics), and other preventative services to the community at large. SCOPE OF WORK In order to provide root canals (endodontics) we are in need of specialized equipment to perform these services with care, precision, safety, and accuracy.  Endodontics is the removal of the nerve from the root of the tooth, and refilled with a material that takes the place of the nerve, so that the patient can retain the tooth, rather than having it removed.  Endodontics may be performed on anterior (front) teeth, and molars (back) teeth.   In order to remove the infected nerve, we are in need of specialized equipment that includes: Files: Used to remove the nerve Obturants: Used to fill the canal where the nerve was removed. The files and obturants must be sterile, flexible, durable, and sturdy enough for the dentist to maneuver in such a small space. In addition, we occasionally need to purchase specialized drills (handpieces) to accompany the use of files, when the dentist prefers to use rotary instruments rather than hand held instruments. METHOD OF AWARD The County intends to award one contract. Award, as determined by the County, will be made to the highest scoring Proposer whose Proposal the County determines is most advantageous to the County based on the described evaluation process and evaluation criteria. Multnomah County strongly encourages the participation of Minority-Owned, Women-Owned, and Emerging Small Businesses and Organizations in providing these services. ESTIMATED PURCHASES The County spends approximately $15,000 annually for endodontics supplies.  The County does not guarantee that any minimum amount of services will be purchased. METHOD OF CONTRACTING A Services Contract form is provided in the Buyer Attachments page showing the standard terms and conditions.  Carefully review the terms and conditions of the Contract.  Additional Contract terms related to this procurement, if any, are set out below in the section entitled Term of Contract.  TERM OF CONTRACT The contract term shall be five (5) years or not to exceed $150,000, whichever comes first. The County shall have the right to terminate the contract on any contract anniversary date. CONTRACT NEGOTIATIONS The County will initiate contract negotiations with the responsive and responsible Proposer with the highest scoring proposal. Multnomah County may, at its option, elect to negotiate general contract terms and conditions, services, pricing, implementation schedules, and such other terms as the County determines are in the County’s best interest. If negotiations fail to result in a contract, the County reserves the right to terminate the negotiations and initiate contract negotiations with the next highest scoring responsive and responsible Proposer. This process may continue until a contract agreement is reached. INSURANCE REQUIREMENTS The insurance requirements are set forth in Exhibit 2 of the Services Contract located in the Buyer Attachments page.  Please review these requirements carefully. MINIMUM PROPOSAL SUBMITTAL REQUIREMENTS At the time of proposal submission proposers must meet the following minimum requirements. Failure to provide any of the required documents or meet any of the below requirements shall result in rejection of the proposal. The Proposal response must be received by the Multnomah County Health Department no later than 4:00 P.M. local Portland time on the proposal submission deadline. Proposer Representations and Certifications At the time of Contracting proposers must meet the following minimum requirements. Failure to provide any of the required documents or meet any of the below requirements shall result in cancellation of the contract Proposers must be legal entities, currently registered to do business in the State of Oregon (per ORS 60.701). Proposers must submit verification that all insurance requirements are met.
