INTRODUCTION AND PROGRAM HISTORYThe National Family Caregiver Support Program (FCSP) was established under the Older Americans Act (OAA) Amendments of 2000 (Public Law 106–501) as the first federally-funded program to support the service needs of family caregivers  This program provides critical services to unpaid caregivers caring for adults with functional disabilities or relatives who are raising children.  These services help delay or avoid entry into a long term care setting and the Medicaid system.  While the Aging Network has always been involved in meeting the needs of care recipients and caregivers, by creating the Family Caregiver Support Program, Congress explicitly recognized the important role that family caregivers occupy in our nation’s long-term services and support system.  Funding for this program is provided in the Older Americans Act (OAA) funding.Aging, Disabled, and Veteran Services Division’s (ADVSD) Family Caregiver Support Program has the following elements: Case Management for Family Caregivers. FCGS Stipends are small grants to family caregivers who are caring for persons age 60 and over, or of any age if the care recipient has Alzheimer’s or a related disease; or for individuals who are grandparents 55 years of age or older who are caregivers of a related child or relative adult with a disability (non-parental). This program is accessed by Family Caregiver Support Program Case Managers of contracted agencies. GOALS AND VALUESA major philosophy of the Older Americans Act  is that preventive and supportive services that help older people to maintain their independence should be available to all older persons and their families. The ADVSD philosophy is that policies, programs and services are established in response to the needs and expectations of participants, and to ensure an outcome of quality service for older individuals and their families.  ADVSD is committed to contracting and partnering with Enhancing Equity contractors that focus on the needs of a particular cultural, racial and/or ethnic group in the entire County and provide services based on community need and organizational capacity.SCOPE OF WORKCase Management for Family CaregiversFamily Caregiver Support Program Case Management is a comprehensive service provided to Family Caregivers who are caring for older adults, or are caregivers of a relative child or children, or are caring for a loved one with Alzheimer’s disease or other dementia. This program also covers non-parental relatives age 55 or older who are caring for a related adult with a disability, or a relative child 0-17 years old. Contractor shall provide case management services including: assessing needs, developing care plans, documenting in Oregon Access, authorizing services, coordinating services, counseling and problem-solving, evaluation of services, advocacy, follow-up and reassessment. This category includes the Star Caregiver intervention.The definition of Family Caregiver includes friends, neighbors and domestic partners who care for someone age 60 or older, or of any age if the person in care has Alzheimer’s or another dementia. Case managers for family Caregivers can access ADVSD’s Family Caregiver Support Program, which provides small grants to family caregivers. Note: Peer support groups for family caregivers are an aspect of Focal Point.This event is seeking proposers to specifically support the Latinx and/or Native American populations throughout the County. The County seeks to support both populations with a culturally specific provider.GENERAL REQUIREMENTSTechnology Requirements for ADVSD Community Services ContractorsAny contractor who provides services to ADVSD Community Services must have an effective and secure computer system and must be able to use basic Microsoft Office Suite programs such as Word and Excel, compatible with PCs.Secure EmailContractors are responsible for protecting PHI as required by HIPAA. Information containing PHI may be transmitted by encrypted email, fax, phone, mail or personal delivery.Data Systems used for ADVSD Community Services ProgramsSeveral different databases are used to record participant and/or service information for ADVSD CS programs.State of Oregon DatabasesThe State of Oregon maintains two databases that are used for ADVSD Community Services programs: Oregon Access and the State Mainframe. Both databases are accessed through a (web- based) portal called Citrix. Contractors performing services for ADVSD Community Services programs must: Sign a data-use agreement with the State of Oregon Identify staff who need access to the systems and request access through ADVSD Notify ADVSD immediately when an individual no longer needs this access Oregon AccessOregon Access is used for recording participant information for these services:  Oregon Project Independence Older Americans Act Case Management Evidence Based Health Promotion Family Caregiver Support Program State MainframeThe State Mainframe is used for authorizing payments to individual Home Care Workers.Universal Client Registry (UCR)This is the ADVSD Community Services internal database. Currently ADVSD CS contractors do not enter data directly into the UCR, except for Transportation services. Contractors providing FCGS and Culturally Specific Meals complete a demographic data form for each participant. Contractors also send service information, generally in Excel format, which is uploaded by ADVSD staff into the UCR.METHOD OF AWARD The County will award a minimum of one (1) and up to two (2) contracts as a result of this Sourcing Event. The County prefers to issue one contract for each culturally specific population, Latinx and Native American. Providers will identify their population in the Proposal Questions and Evaluation.ESTIMATED PURCHASES The anticipated maximum amount of funding for this project is $150,000 over the next five (5) years. The County does not guarantee that any minimum amount of services will be purchased. METHOD OF CONTRACTING A Services Contract form is provided in the Buyer Attachments page showing the standard terms and conditions.  Carefully review the terms and conditions of the Contract. Additional Contract terms related to this procurement, if any, are set out below in the section entitled Term of Contract. TERM OF CONTRACT The anticipated contract period for this service is September 1, 2020, through June 30, 2025. These services may be included in the Community Services Formal Procurement scheduled for 2022, if this happens, contracts resulting from this event may be superseded by the new event.INSURANCE REQUIREMENTS Multnomah County standard insurance requirements are set forth in Exhibit 2 of the Services Contract located in the Buyer Attachments page. Please review these requirements carefully. Specific insurance requirements may be adjusted at the time of contract negotiation depending on the final scope of services. Specific insurance requirements may be negotiated at the time of Contracting. PRE-AWARD RISK ASSESSMENT Successful proposers whose contract award includes federal funding (as identified by a Catalog of Federal Domestic Assistance number) will be subject to a Pre-Award Risk Assessment (which includes an evaluation of financial stability, quality of financial /management systems, experience with federal funds, reports and findings from audits) completed by Multnomah County (if one has not been submitted in the last year) prior to the issuance of a contract. Contractors who fail to submit the required documents will not be eligible for a contract from the County.
